Important Information
Preceptor Agreement Information
Students must complete the Preceptor Agreement Form that is located under Forms & Resources on the RN to BSN Nursing Knowledge Center. This completed form must be uploaded in the Pre-Clinical Canvas course. The earlier the Preceptor Agreement is submitted, the sooner the onboarding process can begin.
- If an affiliation Agreement and/or onboarding is required, you will either be sent an email of what onboarding is required or assigned onboarding to submit in Canvas.
Determine if your clinical facility requires an Affiliation Agreement (AA)
- Ohio University does not require Affiliation Agreements (AA). However, depending on your precepting site, an AA may be required by the facility. Students MUST determine if the facility where precepting hours are to be completed requires an Affiliation Agreement.
- Students should contact their site’s education or human resources department to make this determination.
- Students completing hours at a facility requiring an AA must email Mashawna Hamilton; hamiltom@ohio.edu with the NRSE 4595 Affiliation Agreement Request form. A search will be conducted by Ohio University to determine if there is an existing AA in place.
- If an existing AA is NOT in place and is required by your site, students will be required to complete the AA request form, which can be found under Forms and Resources on the RN to BSN Nursing Knowledge Center.
- **There is no guarantee how long the Affiliation Agreement process can take or that a finalized agreement will be reached if either party cannot agree to the terms.
