- Petition for Reinstatement [PDF] (opens in a new window): This form is for students who have been academically dismissed from Ohio University, College of Health Sciences and Professions
- Re-enrollment Form: This form is for students not currently enrolled who want to return to Ohio University
- Satisfactory Academic Progress (SAP) Appeal Form: This form is used to appeal SAP status.
- Transcript Request Form: You can order your transcripts online using this form. There are also directions if you would like to order your transcript by mail or phone.
- Tuition Appeals and Late Drops: Please contact chspss@ohio.edu to determine is you should complete a tuition appeal or late drop.
- Update of Program Form: This form is used to change your major, or add a minor or certificate.

- To withdraw from the University, you must drop all your classes after the earliest start date of any of your registered classes. Contact your college or regional campus student services office to initiate the withdrawal process. Graduate students must contact the Graduate College.
- WellTrack Boost is a self-guided interactive online therapy program that is free and confidential for all students.
