Report an Incident or Concern

There are times when you may have a concern about a student. Faculty, staff, and students are encouraged to submit an online report related to behavior or an incident of concern.

Please use the appropriate form below to report issues to the appropriate office. These forms should not be used in situations where immediate attention is required.

Do not use these forms to report an immediate threat. If you require emergency assistance, call 911

Concerning Student Behavior

Please use this form through the Dean of Students Office to refer a student whose behavior may appear to be distressed or disruptive and may be a cause for concern.

Student Code of Conduct Violation

Please use this form through the Office of Community Standards to report any incident that violates the Student Code of Conduct. This includes behavior related to alcohol, drugs, harassment, vandalism, disorderly conduct, and hazing. If you have questions about which form to use or if a particular behavior violates the Student Code of Conduct, please consult with the Office of Community Standards prior to completing the form.

Reports of sexual misconduct and academic misconduct should not be reported via this form.

Community Standards and Student Responsibility (CSSR) aims to partner with faculty in addressing concerns of academic misconduct. Faculty and staff may use this form to report incidents of academic misconduct.

Please use this form through University Equity and Civil Rights Compliance to report instances of sexual misconduct (including sexual assault), relationship violence, and stalking.

Campus Climate Concerns

A campus climate concern is any act that appears to be or is perceived to be inappropriate by an individual. Such an act may be motivated by race, ethnicity, age, religion, national origin, sex, disability, gender identity or expression, sexual orientation, veteran status or socioeconomic status.

To be considered a concern, the act is not required to be a crime under any federal, state or local statutes, nor does it have to violate university policy.
