Anthem Releasing New ID cards for plan year beginning July 1, 2021
Anthem has starting mailing new ID cards to employees and covered dependents. Employees and covered dependents will receive new Anthem ID cards in their name, including children. Previously all cards were issued in the cardholder’s name only. The new ID cards are to be used for both the Anthem medical and dental plans. (Previously all cards were issued in the cardholder’s name only, and separate cards were provided for medical and dental plans). Cards were mailed out in mid-June to home addresses and should arrive within 5-7 business days.
- You will need to use the new cards beginning 7/1/2021. Let your providers know that they will need to update their records as Ohio University group number and plan codes have changed.
- The back of the card has new customer service numbers.

PRESCRIPTION COVERAGE: CVS CAREMARK
Continue to use your current CVS Caremark ID cards. CVS Caremark is not distributing new ID cards. Cards are in the employee's name, covered dependents do not receive cards in their name(s).
Electronic cards are available through the caremark.com account or the CVS Caremark app. Visit https://www.caremark.com and create an account to manage prescriptions, comparison shop, view claims and balances, locate a pharmacy, price a medication and more. Due to HIPAA guidelines the cardholder can only see those dependents on their account that are under the age of 18. Dependents 18 years and older will need to register separately. Dependents may grant family access to their account (top of the page – right corner, then under update my profile click on family access).
VISION: ID CARD NOT REQUIRED
Vision coverage does not require an ID card. Simply call a VSP (Vision Service Plan) doctor for an appointment and identify yourself as a VSP member. You aren't required to complete any up-front paperwork or obtain a benefit form. After you've scheduled your appointment, the VSP member doctor will contact VSP to verify your eligibility and plan coverage. The doctor will also obtain authorization for services and materials.
If you're not eligible for benefits at that time, the doctor will communicate that information to you. In addition to your VSP benefits, Anthem Blue Cross & Blue Shield provides an annual preventive vision screening (No deductible 100% reimbursement in-network, No deductible 70% reimbursement out-of-network).

FLEXIBLE SPENDING ACCOUNTS: WageWorks/ HealthEquity Debit Cards
Health Care Flexible Spending Account enrollees may continue to use their WageWorks debit card. Check the expiration date on your WageWorks card before destroying. Only expired cards will be reissued, new cards will include the HealthEquity logo as WageWorks recently merged with HealthEquity. Login to your WageWorks account at https://www.wageworks.com/ or call 1-877-924-3967 to request a new card if necessary. Dependent Day Care Account enrollees do not receive a debit card.
WageWorks/ HealthEquity offers the WageWorks EZ Receipts Mobile App. Check your balances, submit claims, snap and save photos of receipts—all on the go.
