OUAA Sanctioned (Level 1) Event Notification Form

The OUAA Sanctioned Event Notification Form informs the OUAA of level 1 events and requests event promotion services from OUAA.

OUAA Sponsored (Level 2) Support Application 

The OUAA Sponsored Event Support Application requests funding, event promotion, and a level partnership with the OUAA on a proposed level 2 event.

W-9  

This form is used for level 2 sponsored events only. The W-9 form is a Request for Taxpayer Identification Number (TIN) and Certification. It is used to request a taxpayer identification number (TIN) for reporting on an amount paid. You will use this form when a contract is submitted with a vendor, in order for the payment to be recorded for tax season.

SPIF

This form is used for level 2 sponsored events only. The Supplier/Payee Information Form collects information about a new or updated vendor, supplier, or payee. You will use this form when a contract is submitted with a vendor, in order for the vendor to put into OHIO's system and paid.

New Alumni Network Interest Form

The New Alumni Network Interest Form allows alumni to request new Alumni Networks that revolve around shared geographical location or affinity.
