Computer Request Form
OU-HCOM Faculty and Staff:
The Heritage College will continue the faculty/staff central computer replacement program for the 2017–2087 fiscal year. To qualify for a replacement computer, your current computer must be at least 4 years old and out-of-warranty. If you are in need of a replacement computer for the upcoming year, please complete this form. A list of all requests will be evaluated by the Office of Information and Learning Technologies (OILT).
Clinical Campus faculty and staff will follow this same process to request a replacement computer. (If you reside at a Clinical Campus, make sure to select the Clinical Campus drop down option on the form.)
Please have all requests completed by August 7, 2017.
This form allows one to request either a new desktop computer or a new laptop computer. Departments or administrative supervisors will need to decide if an employee’s duties require a desktop or laptop computer.

Desktop Computer:
- A Dell computer will be the standard replacement computer, the specs are i7-3.6GHz CPU, 8GB RAM, 256 SSD HD.
- If you choose an iMac as a desktop computer, you will need to cover the difference in costs (if the cost is higher than the standard Dell of $1,215.00). On the form there will be a place to enter the account number to be used to cover the difference.
Laptop Computers:
- There are three Dell options to choose from; the specs for each will be listed in the form. If you would like to order a docking station ($150.00) or any additional monitors ($130 each) you will need to cover the additional costs--the central purchase process only covers the laptop, not any additional equipment. On the form there will be a place to enter an account number if you choose to purchase the additional equipment.
- If you choose a MacBook as a laptop computer, you will need to cover the difference in costs (if the cost is higher than the standard Dell of $1,215.00). On the form there will be a place to enter the account number to be used to cover the difference.
